Patient Bill of Rights

You have the right to considerate and respectful care.

You have the right to and are encouraged to obtain from your caregivers relevant, current and understandable information concerning the diagnosis, treatment and prognosis of your condition.

You have the right to know the identity of all clinicians involved in your care, as well as when those involved are students, residents, or other trainees. You also have the right to know the immediate and long-term financial implications of treatment choices insofar as they are known.

You have the right to consent to or refuse a treatment, as permitted by law.

Health care institutions must advise you of your rights under state law.

You have the right to every consideration of privacy. You have a right to expect that case discussion, consultation, examination and treatment will be conducted with consideration for your privacy.

You have the right to expect that all communications and records pertaining to your care will be treated as confidential by the facility, except in cases such as suspected abuse and public health hazards when reporting is permitted or required by law.

You have the right to expect the facility will emphasize the confidentiality of this information when it releases it to any other parties entitled to review in formation in these records.
